Pump Bearing Failure Case for New Installation and Lubrication

Story of my life.... 🤦‍♂️ ..I don't know how to explain this to the engineer, we recommended that they change this pump due to outer race defect on DE bearing in May 2024, they did, we opened the bearings and yes, the damage was alarming, on the new pump we took our baseline, all was good, the pump is prone to cavitation due to operational requirements hence the high G's on TWF...just a month after we installed a new pump, boom...again...BPFO, distinct and with high G's on TWF (75G's)....we took a risk and decided to lubricate, trend vibration & temperature changes, and now on the 2nd month, I think I should make a call to change it, the G's are starting to drop on PeakVue, the defect freq. on normal spectrum are starting to disappear and being swallowed by the raised noise floor, I think we are now on stage 4 of this bearing failure, any Ideas how I can explain this? or must I make a call and ask for a chance to explain this after RCA?
