By improving the #reliability of systems, I think one of the most critical conflicts is process-design flaws. It causes many failings on the machines that you can make it less dangerous but never being vanished.
One of those reliability engineer's nightmares is #cavitation in pumps.
Here is a clear hump on the high-frequency vibrations that shows severe cavitation in a BB2 type pump. Increasing the upstream pump's head to the maximum available level is a case of Hobson's choice. Still, a tight parts inspection schedule is necessary; if it doesn't pursue well, the significant damages are irrepressible.
