One of our centrifuge machine electric drive had vibrations above normal levels. During last survey I noticed peakvue graphs for inboard was in the fault zone. We didn’t take readings almost 4 months before the last survey due to short in resources.

After cutting both beatings to do failure analysis. Inboard bearing was flaked and spallled in the inner race and ball elements and outer race. The outboard bearing was showing Alert and fault but no frequencies from the bearing was showing up in graph and after we cut it no defect was observed.

The electric motor drive is 30 hp on VFD drive 60 HZ 4 pole. Belt drive motor
Let me know your thoughts on root cause of the failure.
