When Vibration Warnings Are Ignored—A Costly Mistake
During my latest vibration analysis visit, I identified a critical bearing issue on an AUX Boiler Fan—a failure in progress that could have been prevented.
📌 Key Findings:
🔹 Demodulation spectrum revealed high-amplitude impacts at FTF and BPFO frequencies—clear signs of bearing distress.
🔹 Crest Factor 7.7—pointing to severe shock loading.
🔹 High radial looseness—indicating improper fit, either due to bearing seat wear or inadequate mounting.
🔹 Harmonic patterns confirmed advanced cage instability and outer race deterioration.
⚠ What Happened Next?
Despite my urgent shutdown recommendation, operation refused to stop the fan. One day later, the fan tripped unexpectedly, and maintenance found:
🔻 Broken bearing cage
🔻 Burned bearing housing
🔻 Severe looseness leading to total bearing failure (SKF 22216E)
🛠 Lessons Learned:
✅ Vibration analysis is an early warning, not just a report.
✅ Bearing looseness accelerates damage—addressing it early prevents catastrophic failure.
✅ Ignoring predictive maintenance doesn’t save time; it costs more in downtime, repairs, and risk.
