In gear transmissions, the first damage is usually received by small pinions. Since they are desired to receive the first damage during the manufacturing phase, the material hardness and properties are selected differently than other gears.
Sometimes it may be due to manufacturing or assembly errors. In the example in this post, it is the Agitator pinion gear failure detected in vibration measurements. As a result of the failure, the agitator was sent to service for maintenance.
